Navvis Launches CJR-X Solution as CMS Mandates 90-Day Joint Replacement Episodes

By ·

Key takeaways

  • Navvis announced a CJR-X solution for health systems preparing for the CMS mandatory episode-based model that starts January 1, 2028.
  • The original CJR model generated $58.8 million in Medicare savings in Performance Year 7, but about 60 percent of participating health systems owed repayments.
  • Navvis has supported more than 20 health systems managing over 32,000 episodes annually, with episode cost reductions of 10 to 30 percent.

Navvis announced a new CJR-X solution on October 22, 2026, built to help health systems perform in the Centers for Medicare & Medicaid Services mandatory Comprehensive Care for Joint Replacement Expanded model. The company said the solution combines episode-management operations, post-acute network strategies and physician alignment into a single operating model. The announcement was distributed by Business Wire.

Navvis Builds for the CJR-X Episode

Navvis said the solution helps health system leaders assess episode opportunity and financial exposure, design governance and gain-sharing structures, and coordinate care, discharge planning and post-acute optimization across a 90-day episode. The model covers every Medicare patient undergoing hip, knee or ankle replacement, extending beyond the hospital into skilled nursing facilities and home-based recovery.

The original CJR model generated $58.8 million in Medicare savings in Performance Year 7, while approximately 60 percent of participating health systems owed repayments to CMS, according to the company. Navvis has supported models including CJR, BPCI, BPCI-Advanced, TEAM and Cardiac Episode Payment Models, and was one of 12 original BPCI facilitator organizations.

CMS finalized the CJR-X model on July 31, 2026, as part of the Fiscal Year 2027 Hospital Inpatient Prospective Payment System final rule, moving the model's start date to January 1, 2028. That leaves health systems 15 months to prepare before participation and performance accountability become mandatory nationwide.

"We're continuing to implement this important work in partnership with health system leaders across the U.S. that are investing now in disciplined episode management, physician alignment, and post-acute coordination," said Tim Elliott, CEO of Navvis. "These health systems will be the ones that come out ahead both financially and clinically in episodes of care."

Navvis's Track Record in Episode Management

According to akta.pro company records, Navvis appointed Tim Elliott as chief executive officer on February 24, 2026, returning operational leadership to a founder; he also serves as CEO of parent company Surround Care. The same records show Navvis was named a Leader in the IDC MarketScape: U.S. Value-Based Care Strategic Consulting Services in January 2026, among 14 organizations analyzed.

Navvis has supported more than 20 health systems in managing over 32,000 episodes annually, delivering more than $15 million in savings and episode cost reductions of 10 to 30 percent, according to the company. That work has driven a 50 percent reduction in readmissions, a 30 percent reduction in SNF utilization, a 26 percent reduction in hospital length of stay and a 36 percent reduction in SNF length of stay.

CJR-X Readiness Assessment Requests Open

Health systems can request a CJR-X Readiness Assessment through Navvis's CJR-X solution page. Navvis said it currently supports over 3.1 million patients and approximately 4,600 affiliated physicians across 12 U.S. markets.
